Athletic Physical Examinations Physical Forms

	Fall Sports Physicals 2008-2009
	Boys: Wednesday, August 13, 2008 ~ 8:00 am - 10:00 am Football Soccer Cross Country
	Girls: Thursday, August 14, 2008 - 10:30 am - 12:30 pm Soccer Tennis Volleyball Cross Country Cheerleading
	~Forms must be filled out before the appointment~ ~ If you have gone to your Physican the Forms are due no later than Fiday, August 8, 2008.

The following forms make up the Athletic Packet - this entire Packet must be completed once every 365 days along with a physical examination in order to participate in athletics. These forms are available at the school or can be downloaded by clicking on the links below.

Athletic Physicals Procedures and Release Form - Green Form
Athletic Pre-Participation Physical Evaluation Form - Yellow Form
Physical Examination Form White Form completed by medical provider
Athletic Application and Permission Form - Blue Form (revised)
Parent/Guardian Notification Letter - Pink Form
NJSIAA Policy on Banned Substances - Gold Form
Student Random Alcohol and Drug To-Consent-Form - Ivory Form

For subsequent sports, parents and students need to complete and submit the following forms to the school nurse:

Part A: Health History Questionnaire (Yellow Form)
Athletic Application and Permission Form (Blue Form)
Student Random Alcohol and Drug To-Consent-Form - Ivory Form

The New Jersey State Department of Education allows School physicals in school or students may obtain their physicals with their own healthcare provider. The results will be recorded on a school form, which will be returned to the school nurse.

Students who do not return their forms by the appropriate deadline will not be able to participate in their activity until all forms are returned and approved.

Physicals for 9th Graders and New Students

9th graders and all other students new to the Hanover Park Regional High School District must complete a physical examination by their private healthcare provider during the first year of attendance. An examination conducted within 365 days of the start of school will be accepted provided it is properly documented. Forms for the entry physical are available from the school nurse. Students who do not submit proof of this will be scheduled for a physical with the school physician.

Students currently in 9th grade who have had a physical for athletics need not have another physical. The same is true for students new to our district who have had an athletic physical.
