Physicals for 9th Graders and New Students

9th graders and all other students new to the Hanover Park Regional High School District are required to complete a physical examination by their private healthcare provider during the first year of attendance. An examination conducted within 365 days of the start of school will be accepted provided it is properly documented. If your child is/ will be participating in a sport this school year, the athletic physical will apply for the 9th grade physical requirement.

Forms for the entry physical are available on our website. Or you may use the form from your child's physician.

Students who do not submit proof of a physical will be scheduled for a physical with the School Physician.
