NJSIAA Spring Season Athletic Clearance Procedures
** 2024 Sports- REGISTRATION/ PHYSICAL FORMS
Hanover Park High School is now using the convenience of online registration for athletic programs, using a secure online registration platform called FamilyID. FamilyID is a registration platform that provides you with an easy, user-friendly way to register for our athletic programs. When you register through FamilyID, the system keeps track of your information in your FamilyID profile. At any time, you may login to www.familyid.com to update your information and to check your registration.The majority of forms for the 2024 NJSIAA Spring Season can be submitted electronically on the FamilyID Website. Registration is to be completed each sport season by a parent/ guardian.
Students signing up for Park Boys Lacrosse and Park Girl's /Boy's Golf must do this through the Hanover Park Family Id Portal.
Students signing up for Park Girl's Lacrosse must do this through the Whippany Park Family ID Portal.

Please use the following procedure to sign up for all Winter sports
Print out forms/schedule a physical with your physician.
Spring Sport Physicals / Family ID Registration are due by
March 4, 2024
Please be advised that physicals need to be within 365 days of the start of practice.
NJSIAA Start Dates for all sports are as follows:
March 11th- Boys Tennis, Baseball & Softball (pitchers and catchers)
March 14th- Spring Track, Park Girls/ Boys Golf, Park Boys / Girls LAX, Baseball & Softball
If you are having difficulties with scheduling an appointment with your own primary care physician, you can make an appointment to go to the facility of our school physician, American Family Care - Urgent Care. Athletic physicals at American Family Care - Urgent Care must be done between
February 12, 2024- March 1, 2024 (Mon- Friday)

To make an appointment for a school athletic physical, please use the link below to register for an appointment.
Step 1:THE FOUR-PAGE PREPARTICIPATION PHYSICAL EVALUATION PACKET, which includes the History Form, Athlete with Special Needs Supplemental History Form, Physical Examination Form, and Clearance Form will be the only forms that will be submitted to the Health Office at Hanover Park High School. If you have been diagnosed with asthma, diabetes, or carry an EpiPen, additional forms must be signed by your physician and returned with your completed 4 PAGE Packet. These forms can be found and printed off the FAMILYID website.
Step 2: Register on Family ID ( Must be completed for each season you play a sport or weight room participation)

FamilyID REGISTRATION STEPS:
Click on the blue "CREATE ACCOUNT" link in the top right hand corner of FamilyID.com.
Create your secure FamilyID account by entering your name, email address twice, a password twice, and checking the "I agree to the FamilyID terms of Service" box. Then select the green 'Create Account' button.
You will receive an email with a link to activate your new account. (If you do not see the email, check your spam or junk folders.)
Click the link to activate your account.
This will bring you to the FamilyID Dashboard.
